Seton Health's Readmission Team Honored with Quality Award from IPRO
TROY, N.Y. -- Seton Health / St. Mary’s Hospital was honored with a Quality Award from IPRO on June 8. IPRO, the Medicare Quality Improvement Organization for New York State, distributes the awards annually to recognize individuals and organizations that demonstrate a commitment to improving the quality of care provided to New York’s Medicare beneficiaries.
Seventeen health care organizations and professionals throughout New York State received the recognition from IPRO. The awards were given during the 26th Annual Meeting, held at LaGuardia Marriot Hotel in East Elmhurst, N.Y.
The Readmission Team won for its excellence in multidisciplinary teamwork that spans the inpatient and outpatient continuum of care. Due to the team’s impressive work, the readmission rate dropped an astonishing 6% at St. Mary’s Hospital and 32% in Home Care. The reduced rate is due to patients being assisted in gaining the knowledge and help needed to manage their care at home and prevent costly recurrence of their illness.
