What are Anticoagulation Medications? 

Anticoagulation medications such Coumadin® (warfarin) are prescribed to thin your blood and prevent blood clots. While in most cases the benefits of blood thinning medications far outweigh their side effects, it is extremely important that patients taking these medications be monitored closely on a regular basis to avoid serious complications. 

About St. Peter’s Health Partners Anticoagulation Clinics 

Our clinics are staffed by pharmacists, nurses and medical professionals specially trained in the management of both oral and injectable anticoagulation medications. We will work closely with your primary health care provider or health care specialist.

Our clinics are located at Albany Memorial Hospital in Albany, Clifton Park Family Health Center in Clifton Park, Seton Health in Latham and the Massry Center in Troy. 

Services include:

  • Point-of-care (finger-stick) testing
  • Anticoagulation medication adjustment
  • Close monitoring and treatment for side effects
  • Patient education
  • Screening and counseling about food and drug interactions
  • Instruction on preparation for surgery or other invasive procedures 

Enrolling in Our Clinics

Your primary health care provider or specialist must provide you with a referral to our clinics. We require that your provider send us your current medication list, including your anticoagulation drug history and current medical condition list prior to your first appointment. 

What to Bring to Your First Appointment

On your first appointment, we ask that you bring the following: 

  • Your current anticoagulation medication dosing instructions
  • All of your medication bottles
  • All supplements and over-the-counter products that you are taking

It is important that you bring a list of any medication changes to each appointment. 

Insurance and Billing 

Please contact your insurance provider to find out if there is a required copayment. If you need to cancel an appointment, please call us at (518) 268-6115.
